CONNECT is a decision aid for people with angina. CONNECT provides information and asks questions to help you make a choice about your heart test and treatment.

What are my options?
  1. Take heart medicines.
  2. Take heart medicines and have a coronary angiogram test plus treatment with coronary angioplasty, if suitable.
 You may want to look through CONNECT with your family, friends, or carer. Look out for the pencil symbol. This shows you where there are questions to answer about your symptoms, treatment preferences, and what’s important to you. Your answers will be shown in the summary section. You can share your summary in the next consultation with your heart nurse or doctor.
